A7s mk1 + Kowa B&H 2x Anamorphic + FM Lens

I really like the look of this video:

There are technical notes in the description but they are very vague and as a beginner I would be grateful for any help interpreting them. I am trying to achieve the same look and am looking to buy the lenses but need more info.

Ok so the main equipment listed is the A7s, a MODDED FM lens and the Kowa B&H 2x Anamorphic lens as well as a standard spherical lens. 

1. Anyone know how he could have modded the FM? Why would he need to do that in the first place?

2. How did he mount the lenses to the FM lens? I heard it is not that easy

3. What does he mean by  "full frame anamorphic from the A7S codec"? As in, what did he have to do to get the full frame image?
